🔴 Vasculitis | التهابات الأوعية الدموية:-
📚 Definition of vasculitis:-
Is a general term for vessel wall inflammation.
هو إلتهاب جدران الأوعية الدمية.
ويتميز بالآتي (Characterised by):-
- Inflammatory.
- Necrotizing.
يحد الـ vasculitis في الـ arteries, والـ veins لكن: more COMMON is arteries.

📚 Etiology of vasculitis:-
توجد Two Mechanisms يحدث
بسببهم الـ Vasculitis:-
✒️ 1. Immune Mediated inflammation:(non infectious):-
تحدث بسبب إضربات في الـ Immune system.
(autoimmune disease).
يقوم الجهاز المناعي بمهاجمة
الـ Endothelial of blood vessels wall.
(this mechanism is more common).

✒️ 2. Direct invasion of vascular wall:-
يتم مهاجمة الـ wall of the vessels بـ
Infectious pathogen,
وهنا يكون infectious
يحدث infection يسبب vasculitis.

✒️ 3. Which mechanism is more common:-
أحياناً يكون الـ vasculitis مصاحباً لأمراض أخرى على النحو التالي:
- يكون الـ 👇
Direct invasion Associated with infective endocarditis.
- ويكون الـ Immune mediated with RS, SLE.
(Rheumatoid arthritis, systemic lupus erthromstosis).

📚 Symptoms of vasculitis:-
✒️ General features of vasculitis:-
- Fever.
- Myalgia.
- Malaise.
- Arthralgia .
✒️ Specific features of vasculitis:-
Depends on organ supplied by affected vessel:-
إذا كان الـ Vasculitis في الـ Renal artery, ستكون
الـ signs & symptoms
في الـ Kidneys
(او لها علاقة بالـ kidney ) مثل:
- Oliguria (decreased urine output).
- Hypertension.
( Because kidneys play a major role in blood volume regulation)
سنفصل ماذا يحدث في كل عضو لكن دعنا نتكم عن الـ Classification.

📚 Classification of vasculitis:-
👉 Most common is classification according to size of vessels involved.
👉 This classification divide vasculitis to three:
🩸 - Large vessels vasculitis.
🩸- Medium vessels vasculitis.
🩸- Small vessels vasculitis.

🩸 1- large vessels vasculitis:-
ويشمل مرضين مهمين:
👉 Giant cell arteritis (GCA).
👉 Takayasu arteritis.

📚 Giant cell ِArteritis (GCA):-
Is a group of vasculitis affect branches of carotid artery.
- يمكن ان يحدث في اوعية دموية اخرى مثل aorta, axillary artery لكن
most common
في الـ carotid.**
- الـ temporal branch من الـ Carotid هو الاكثر شيوعاً في الاصابة بالـ
Vasculitis.
- يؤدي هذا الالتهاب لأعراض مثل الصداع (severe headache)، اذا تأثر الـ
temporal branch of carotid.
- اما اذا كان الـ Vasculitis في الـ ophthalmic artery يسبب
Visual disturbance، واذا لم يُعالج ستحدث ischemia، وقد تؤدي الـ ischemia لـ complete blindness.
- الـ GCA غالباً تصيب كبار السن More than 50 years, ويصيب الـ females اكثر من الـ males
- في الـ Investigation يكون High ESR
- ناخذ biopsy نجد فيها
Giant cell granuloma in blood vessels elastic lumina وهذا سبب تسميتها GCA.
